Photo and Video Consent and Release Form

I, hereby grant to Sparknify LLC, the irrevocable right and permission to copyright, use, publish, alter and modify the photo or video that Sparknify LLC has taken or may have taken of me or my child for any lawful purpose, including, but not limited to, advertising, display, illustration, promotion, publicity and trade. In this regard, it is agreed that I waive any and all rights to royalties, copyrights or other compensation arising out of or related to the use of the video. Furthermore, I understand that the videos may be modified in any way, and I hereby waive any right to inspect or approve any final product using my image.
